The Challenge

NHS West Yorkshire ICB’s Leeds locality was facing a significant backlog of AACC reviews across residential and domiciliary care settings – a backlog that was driving up financial pressure.

The risks were mounting:

  • Many Fast Track care packages no longer reflected the current needs of individuals
  • Resources risked being used inefficiently
  • Capacity to support new referrals was reduced

What Leeds needed:

  • Improved review performance
  • Full compliance with National Framework guidance
  • Smarter, more efficient care provision – without losing sight of the person at the centre.

The Solution

Liaison Care partnered with the Leeds AACC team to deliver clinically-led CHC reviews across residential and domiciliary care placements, including Fast Track, 1:1, enhanced packages, and complex cases. This included:

  • Dedicated team: Our clinically-led practitioners and Social Workers took on the backlog, freeing the ICB to focus on new referrals, mobilising within three weeks.
  • Clinically robust assessments: Every review focused on individual needs, clinical evidence, and National Framework compliance.
  • Quality assurance: All cases went through thorough checks, with recommendations shared and clinically verified by the ICB within 48-hours, showing exceptional quality and excellence.
  • MDT oversight for complex changes: Where needs had significantly shifted, MDT meetings kept decisions transparent and person-centred.

As a result, the ICB gained assurance that care packages remained appropriate, sustainable, and aligned to patient needs.

The key outcomes included:

  • 413 reviews completed – robust, person-centred, and fully compliant with the National Framework.
  • Increased capacity for the ICB, relieving backlog pressure and providing lasting assurance.

In addition, two members of Leeds’ CHC team were invited to join the new Postgraduate Diploma in Continuing Healthcare course sponsored by Liaison Care, to help support ongoing internal skills.
